• Claim form – stamped and signed.
• Certified death certificate – the serial number on top must be clearly visible.
• 
  Certified Id copy of deceased and main member must be clear.
• BI 1663 Forms – completed.

If the death certificate is handwritten, please provide a letter from home affairs explaining why the handwritten certificate was issued.

If it’s the death of a stillborn, please provide BI 1663 forms and a handwritten certificate.

If death is unnatural, the police officer investigating the case must complete the police report and must be attached.
